Job responsibilities

The Neighbourhood Care Clinician role will work with Primary Care Network (PCNs) practices to proactively manage patients living with frailty and other long-term conditions. In conjunction with the wider health, social and voluntary care workforce this role will lead and facilitate patient centred coordinated case management approach across primary, community and secondary care for people who are most vulnerable to and at high risk of admission to hospital.

Working autonomously to a high standard of clinical care, the clinician will be responsible for liaising with the PCNs to identify patients living with frailty who would benefit from an intervention to either prevent further deterioration of their frailty or other long-term conditions and to prevent hospital admission.

Using the medical model to assess, diagnose and prescribe for acute medical conditions, the clinician will use a holistic approach to consider methods and interventions to prevent recurrence of problems and complete referrals to appropriate members of the wider multidisciplinary team.

Following clinical assessment using comprehensive geriatric assessment either in the patients home or clinic environment, multi-disciplinary patient pathways will be designed to support patient choice, improve quality of life, promote self-management and assure early intervention through proactive provision of care in or close to the patients own home as possible.
