We have an exciting opportunity for a passionate and caring individual to join our dynamic team.

Main duties of the job

The postholder will be part of the Wessex Clinical Van service, providing outreach treatment pathways for viral Hepatitis across Hampshire, Dorset and the Isle of Wight.

This will include health screening and assessment, other health and wellbeing treatment, signposting and onward referral to the relevant services. Our service users are at risk of have having life limiting/life restricting co-morbidities and are at risk of earlier death.

This service will aim to increase access to Hepatitis C treatment pathways and other Health and Wellbeing services and play a significant role in identifying illness early with the aim of accessing the right treatment at the right time.

Job responsibilities

To support the Wessex Clinical Van Coordinators in the delivery of the service supporting/leading on testing and treatment clinics, testing events and engagement work.

To promote the uptake of Hep B, C and HIV testing across Wessex.

To test service users for BBVs and provide pre/post-test discussions

To support stakeholder organisations in engaging service users in getting tested and accessing treatment for BBVs if appropriate.

To analyse BBV results and record them on ILLY Care Path and other organisational recording systems.

Maintain pathways into Hepatology and HIV services

To provide support to individuals re: planning and accessing treatment for Hep B, C and HIV Supporting service users through their BBV treatment journey from beginning to end.

Support the Wessex Clinical Van Coordinator in training events, peer to peer talks, presentation and attending conferences to promote the work that the Wessex Clinical Van does.

To work with the Wessex Clinical Van Coordinator in monitoring outcomes for active projects within the service

Person Specification Qualifications
  • Foundation Degree / HNC in related field which may include counselling, social work, nursing, psychology, human resources, social studies, or equivalent experience
Knowledge
  • Excellent knowledge of blood borne viruses, health inequalities, physical/mental health screening and assessment
