Are you a band 5 registered nurse with an interest in diabetes? Do you want to work in a motivated, forward‑thinking team?

We are looking for motivated individuals to join our team on a full‑time basis. With support, guidance and mentorship we will help develop your knowledge and skills across all aspects of our community diabetes service, working towards sign‑off of competencies to become a band 6 diabetes specialist nurse (DSN) within an anticipated 6 to 12 month period.

Entering the service at band 5 level, we will work with you to develop skills and knowledge to complete the competencies required to work as a band 6 diabetes specialist nurse within our community settings. We anticipate this will take between 6‑12 months.

You will gain the skills and knowledge to help support and manage people with diabetes referred to the service within a defined population as well as provide diabetes advice and guidance to other health care professionals.

Main duties of the job

You will be supported and mentored to develop skills and knowledge to actively participate in service provision for people with diabetes, providing specialised clinical expertise, leadership, education and practice development within our defined geographical boundaries of North and West Hampshire.

You will work as part of the multidisciplinary Diabetes team providing specialist advice, demonstrating a sound knowledge of the issues related to the identification, assessment, treatment and management of this clinical group, particularly those with complex needs.

In this role you will give specialist advice to GPs and other health professionals about diabetes, especially with reference to titration of medication. You will monitor and improve standards of care through supervision of practice, clinical audit, evidence‑based practice, teaching and supporting professional colleagues and the provision of skilled professional leadership.
