Een zorgorganisatie in Welkom zoekt een Verpleegkundige om deel uit te maken van het team Langdurige Zorg Thuis. Je bent verantwoordelijk voor de coördinatie van zorg op cliëntniveau en helpt cliënten zelfstandig en veilig thuis te wonen. Als Verpleegkundige voer je intakegesprekken en signaleer je veranderingen in de zorgbehoefte. Het aantal uren is tussen de 24 en 28 per week.
Interessante arbeidsvoorwaarden zijn inbegrepen, inclusief een vast contract en ruimte voor ontwikkeling.
