Responsible To: Aged Care Coordinator/Senior Case Manager

POSITION SUMMARY

The Case Manager is responsible for monitoring the individual needs of clients and their service provision. Key responsibilities include client document and liaison with carers and families. This is a hands on position where issues can arise at short notice that require the Case Manager to be flexible, adaptable and resourceful in translating standards and improvement opportunities into best practice.

STANDARD REQUIREMENTS

FUNCTIONS
  • Walk beside and be a ‘Malpa’ or companion for dialysis patients and their families.
  • Engage in the delivery of appropriate, innovative and sustainable services for Malpa Aged Care.
  • Work with thepa team to deliver direct care services including transport, personal care, social and cultural activities.
  • Advocate for patients and protect the rights of the individual, assisting them to have their voices heard in discussions regarding their health, housing, financial, family and cultural needs.
  • Work as a team to ensure the daily operations, functions and responsibilities of the Malpa Aged Care team are achieved.
  • Communicate and work with key stakeholders internally and externally to ensure holistic quality client services are provided.
  • Work collaboratively with the clinical team to ensure patients can engage in return to country trips, bush trips and short and long-term dialysis on country.
  • Participate in quality audits and evaluation activities.
  • Effectively utilize the client management system and comply with data collection/reporting requirements.
  • General administration, reception and other tasks as required to support the activities of the organisation
  • Promote a safe work environment in accordance with legislative requirements, adheres to Work Health Safety guidelines, and reports all incidents and near misses.
  • Other duties as required within trained capacity and level.

Qualifications, Skills & Experience
  • Certificate in IV in Community Services and/or relevant qualification and/or relevant experience.
  • Demonstrated experience in the delivery of Aged and/or Disability Care Services, associated programs, Industry standards and reforms.
  • Demonstrated computer skills competency, including Microsoft Office software (particularly Word, Excel, and Outlook) and an ability to learn new software.
  • Understanding and experience of cultural protocols, together with a willingness to work respectfully in accordance with Indigenous groups & cultures.
  • Current Senior First Aid and CPR Certificate or willingness to obtain
  • Drivers’ License C Class minimum
  • 3-4 Years experience in related role
