Role Overview

CARE Team (Complex, Assessment, Risk, Empowerment) – Be the advocate for independence. Deliver holistic, strengths‑based support to adults in the community. You’ll assess complex needs, manage risk, and empower individuals to live safely and well. Build strong local networks and become a trusted presence in people’s lives.
