Overview
Siouxland Community Health Center has an opening for: RN Health Coach.
1 – Full-time opening (4 day work week with one 12 hour day)
Successful candidate must have solid experience & advanced knowledge of:
- Minimum of RN license, BSN preferred.
- Current BLS and Mandatory Reporter certifications.
- Clinical office and case management experience preferred.
- Certified Diabetes Educator certification or ability to obtain within 6 months of employment.
- Experienced RN with a background in case management who is self motivated and capable of working with others to improve patient care.
Successful candidate must be able to perform primary functions of position:
- Educate patients on chronic disease management and prevention.
- Assist patients with identifying and setting proper self-management goals including coordination to determine an appropriate goal achievement.
- Contact insurance companies for preauthorization/precertification approvals for doctor’s visits, procedures, diagnostic test, etc. Including maintaining detailed records in the EMR of all preauthorization activities.
- Instruct diabetic patients on proper use of diabetic meter, insulin instructions, and knowledge of downloading meter results.
- Promote public awareness and education in the preventative health habits of SCHC patients via proactively assuring proper screenings are completed.
- Collaborate with provider teams on a regular basis, ensuring to be an active part in daily huddles.
- These functions are not all inclusive.

QualificationsLicenses & Certifications
- RN license (BSN preferred).

Education
- Associates or better in Nursing.
Experience
- Clinical office and case management experience preferred.
