Responsibilities
- Assists students in achieving their individual transition and Individualized Education Plan (IEP) goals using teacher-designed instruction.
- Implements and supports teacher-directed lesson plans.
- Conducts small group instruction.
- Supports and trains students in the use of the public transportation system.
- Supports teacher-directed learning to help students overcome barriers, build self-esteem, and develop positive attitudes.
- Monitors workplace and student participation to ensure the development of work maturity skills.
- Provides positive work exploration experiences in collaboration with a job site teacher.
- Maintains effective working relationships with job sites under teacher supervision.
- Models essential employment skills, e.g. appropriate dress, communication, punctuality, professional relationships and boundaries, etc.
- Communicates relevant job site, community, and classroom information with the teacher.
- Interacts positively and effectively with students and parents under teacher supervision.
- Communicates professionally with staff, teachers, administration, and related service providers.
- Maintains strict student confidentiality.
- Follows the proper chain of command when relaying information.
- Understands and implements plans related to student-specific needs, e.g. health care plans, behavior intervention plans, special interventions, etc.
- Collects student data consistently, following teacher-directed plans.
- Monitors and records student progress accurately, including pre- and post-test data collection.
- Reinforces, models, and teaches appropriate behavior.
- Implements and follows Behavior Intervention Plans (BIPs) as written.
- Provides behavioral support and accommodations.
- Maintains a safe and supportive learning environment.
- Implements Positive Behavioral Interventions and Supports (PBIS).
- Attends required professional development opportunities related to student support, behavior management, job coaching, and monitoring.

- Ability to effectively manage students at jobs sites.
- Ability to monitor student progress daily.
- Basic reading, writing, math, organization, record keeping and computer skills.
- Requires high school diploma or equivalent.

- Makes short-term decisions to support students within program guidelines and teacher-established parameters.
- Interacts positively and communicates effectively with teachers, students, staff, and administration.
- Oversees the safety of students when out in the community.
- Work takes place in a typical school environment, both indoors and outdoors.
- Supervision of students requires both indoor and outdoor responsibilities in all types of weather, temperature, and air quality.
- May experience stress from a disruptive student and/or groups of students.
- While performing the duties of this job, the employee is regularly required to speak and/or hear.
- The employee is frequently required to stand; walk; reach with hands and arms and stoop, kneel, and crouch.
- The employee is occasionally required to use hands to handle or feel objects and must occasionally climb or balance.
- Specific vision abilities required by this job include close vision, distance vision, color vision, peripheral vision and depth perception.
- The employee must frequently lift and/or move up to 25 pounds and occasionally lift and/or move up to 50 pounds.

Job Summary
The Transition Assistant supports adult students with disabilities (ages 18-22) in building skills required for independent living, daily living, job readiness, self-advocacy, and leisure activities. The incumbent assists the teacher in providing large and small group instruction, data collection specific to long-range transition goals, behavior support, and other duties as assigned by the teacher.
