Overview
Certified Nursing Assistant - SNF - Day Shifts (may be eligible for $1,000 sign on bonus) at Barton Health.
Summary Of Position:
Assists licensed staff in the provision of basic nursing and therapy care in Skilled Nursing, using the application of basic skills, restorative care, and established policies and procedures to ensure safe and effective care. Services are provided based on the ages of the population served (adult through geriatric, 65 years or older) in collaboration with the multidisciplinary care team. The CNA SNF supports the nursing process by collecting resident care data, reporting findings, and assisting the team in meeting resident care goals.
The CNA SNF is under the supervision of Skilled Nursing licensed staff and therapists and reports to the Skilled Nursing Director or designee.
- Provide consistently exceptional care at all times as directed by licensed nursing & therapy staff.
- Perform hourly rounding on all assigned residents on every shift.
- Collect resident care data at designated intervals and report findings to licensed staff.
- Recognize and report relevant resident information and any changes in condition or needs to licensed staff.
- Notify charge nurse of feeding problems, reassessment needs, mobility issues, or referrals.
- Utilize time, equipment, and supplies efficiently and safely; complete all assigned duties during the shift.
- Provide personal care assistance to residents, encouraging self-care and independence while offering help as needed: bathing, bathroom/bedpan, oral care, AM/PM care, positioning, dressing, ambulation & transfers, eating, and transport.
- Assist with routine physical therapy treatments under direction of the therapist and charge nurse (Range of Motion, strengthening exercises, etc.).
- Ensure placement of restorative devices and assist with bowel/bladder program as needed.
- Perform basic procedures within CNA scope of practice (e.g., simple dressings, specimen collection).
- Assist residents with nutritional intake and document care appropriately.
- Communicate safety information to the charge RN and follow infection prevention measures at all times.
- Document all aspects of resident care accurately and timely; ensure proper use and documentation of supplies and equipment.
- Demonstrate knowledge and skills appropriate to the age of residents served and comply with regulatory/licensing standards.
- Adhere to hospital and departmental policies regarding attendance and punctuality; respond to department needs as necessary.
- Education: Completion of an accredited Nursing Assistant program
- Experience: Previous long term care experience preferred
- Knowledge/Skills/Abilities: Experience or specialty training in vital signs, bed making, and resident transfer techniques; strong computer skills for patient data entry and notes; proficient English (bilingual abilities preferred)
- Certifications/Licensure: Current CA Nursing Assistant Certification;
Current CPR/BLS certification by the American Heart Association (BLS Provider) or ability to obtain prior to start date

- Operate and maintain equipment, request repairs as needed, and manage supplies.
- Demonstrate knowledge of age-appropriate care and regulatory standards.
- Adhere to attendance and punctuality policies.
- Respond to department needs by performing other duties as necessary.
