Summary of Position:
Assists licensed staff in the provision of basic nursing and therapy care in Skilled Nursing, using the application of basic skills, restorative care, utilizing established policies and procedures, ensuring safe and effective care. Services are provided based on the ages of the population served -- adult through geriatric, 65 years or older -- and in collaboration with the multidisciplinary care team.
Demonstrates the skill necessary to meet the needs of all residents served in Skilled Nursing through support of the nursing process; collects resident care data, reports findings, assists team in meeting resident care goals. The Certified Nursing Assistant SNF is under the supervision of the Skilled Nursing licensed staff and therapists and reports to the Skilled Nursing Director or designee.
Education:
- Completion of an accredited Nursing Assistant program
Experience:
- Previous long term care experience preferred
Knowledge/Skills/Abilities:
- Experience or specialty training required in vital signs, bed making and resident transfer techniques
- Strong computer skills to complete patient data entry and notes
- In compliance with patient safety standards, must be able to effectively communicate in English;
Bilingual abilities preferred
Certifications/Licensure:
- Current CPR/BLS certification by the American Heart Association (BLS Provider) or ability to obtain prior to start date

Essential Functions
- Provide consistently exceptional care at all times.
- As directed by licensed nursing & therapy staff, provides care in a manner that is safe and sensitive to the rights of each resident.
- Performs hourly rounding on all assigned residents consistently on every shift
- As directed, assists the care team by collecting resident care data at designated and appropriate intervals on all assigned residents. Reports finding to licensed staff.
