• Provide nursing care to patients in their homes through assessment, development, implementation, and evaluation.
• Properly comply with physician orders in a timely manner; chart, audit, and demonstrate compliance.
• Assess patient acuity accurately and promptly; communicate via secure messaging with physicians and case managers.
• Independently prioritize and complete routine assignments consistently, accurately, and within guidelines under RN direction.
• Complete patient care activities within scheduled frequencies; obtain daily schedule via EMR application.
• Ensure assessment, planning, intervention, and evaluation of patient care reflect policy, procedures, and standard of care with complete documentation in EMR.
• Update patient plan of care as needed and document appropriately.
• Teach patients and families based on identified health needs; audit teaching goals.
• Report patient problems to primary nurse or management with solutions; document clearly.
• Communicate pertinent patient information and plan of care to primary nurse; obtain physician orders as needed.
• Perform additional duties and projects as assigned and demonstrate flexibility.
• Adhere to HMH's organizational competencies and standards of behavior.
• Lifts a minimum of 50 lbs., pushes and pulls a minimum of 50 lbs., and stands a minimum of 8 hours a day.

• Graduate of an LPN program or equivalent.
• Six (6) months of experience.

• Home Care experience.

• Licensed Practical Nurse.
• BLS/CPR certification from the American Heart Association, required within 60 days of hire.

Minimum rate: $32.40 per hour. Compensation may vary based on market data, experience, education, and other factors.
