Hackensack Meridian Health helps patients live healthier lives while fostering a collaborative culture among team members. This role supports patients’ physical and emotional well‑being through comprehensive social work services delivered in home settings.
Responsibilities- Render professional social work services through assessment, planning, implementation, and evaluation of patients’ social and emotional needs in accordance with organizational policies and applicable standards.
- Maintain practice standards per NJ Department of Health, NJ Social Work Code of Ethics, The Joint Commission, and HMH policies.
- Ensure safe and effective patient outcomes by applying clinical judgment and documenting care in collaboration with the interdisciplinary team.
- Conduct initial psychosocial and bereavement assessments to inform care plans, updating as needed and documenting risk factors.
- Provide counseling and education to patients and families regarding coping strategies and recovery or terminal care needs.
- Document interventions and plans in a timely manner, aligning with organization timelines.
- Communicate relevant patient information and care plans to primary nurses and team members, and coordinate case conferences.
- Report care issues and propose solutions to clinical leadership, maintaining ongoing communication with supervisors.
- Assist patients and families in understanding medical recommendations and navigating decision‑making processes.
- Identify and connect patients and families to community resources for end‑of‑life and discharge planning; follow up to ensure implementation.
- Perform additional duties and projects as assigned, demonstrating flexibility across all responsibilities.
- Adhere to HMH's Organizational Competencies and standards of behavior.
- Perform physical duties: lift or move a minimum of 10 lbs., push or pull 10 lbs., and stand for at least 4 hours per shift.
- Master’s Degree in Social Work from an accredited program or a Doctorate from an accredited institution.
- One year of relevant professional experience.
- NJ Licensed Social Worker or NJ Licensed Clinical Social Worker.
$29.33 Per RVU, based on services provided. The posted rate is a minimum estimate for visiting patients in their homes, in accordance with the New Jersey Pay Transparency Act. Pay for non‑clinical assignments will be discussed during the interview.
