• Provide in-home, para-professional services necessary for caring for patients' personal needs under the direction of the RN.
• Perform and/or assist with personal care activities (baths, showers, skin and hair care, oral hygiene) and follow the care plan with universal safety precautions.
• Notify the RN of changes in patient condition promptly and document appropriately.
• Record all required information on the patient chart accurately and ensure documentation adheres to organizational protocols.
• Assist patient with transfers and ambulation; perform range-of-motion exercises; ensure activities are performed safely and with proper body mechanics.
• General housekeeping of the patient’s area and changing bed linens; assist family with light housekeeping tasks (dusting, vacuuming, damp mopping, etc.).
• Assist client with self-administration of oral medications and remind patient when to take them.
• Prepare simple meals per dietary instructions and assist with eating if necessary.
• Assist with Foley catheter and colostomy care to ensure hygiene and skin health around stoma; perform peri care and measure urine as required.
• Demonstrate flexibility with job responsibilities in all areas and perform other duties as assigned; adhere to organizational competencies and standards of behavior.
• Lift, push, or pull a minimum of 50 lbs. and stand for extended periods as required.

Licenses And Certifications Required

• Homemaker - HHA Certification.
• Valid Driver's License from a USA state.

Minimum rate of $18.36 hourly. The posted pay reflects a reasonable good faith estimate of the minimum base pay at the time of posting and does not reflect the full value of our total rewards package. Some positions may be eligible for performance-based incentives, bonuses, or shift differentials.
