OverviewWe are seeking a Speech Language Pathologist to join our team Part Time or PRN depending on therapist preference. This position provides direct patient care to primarily adult and geriatric patients and involves evaluation, care plan development, and delivery of treatment interventions related to swallowing disorders, dysphagia, and communication disorders, in accordance with state practice guidelines.
Responsibilities- Provides screenings and evaluations to determine requirements for skilled speech therapy intervention relative to patient’s prior level of function, including swallowing disorders, dysphagia, and communication disorders.
- Develops and implements treatment plans that focus on functional and measurable outcomes, based on physicians' orders and adheres to professional standards of practice.
- Monitors patients' progress and adjusts treatment plans as needed.
- Maintains accurate and up-to-date patient records.
- Collaborates with, coordinates, and participates in training programs for community staff, patients, families, and rehabilitation staff.
- Participates in continuous quality improvement activities.
Education and Experience: Successful completion of an accredited Speech Language Pathology program.

- License and Certification:
- Current valid Speech Language Pathologist license to practice in appropriate state.
