Provide one-on-one guidance, support, education, referrals, coordination of care, and other assistance to patients as they move through the healthcare continuum.

MINIMUM QUALIFICATIONS EDUCATION, CERTIFICATION, AND/OR LICENSURE:

High School Diploma or GED equivalent

Develops relationships with participating physicians and service providers.

Liaising with medical providers.

Guiding patients to appointments

Coordinating the logistics for plan adherence-appointments

Supporting patient family members

Demonstrate effective communication skills with all stakeholders (e.g., patients, employer staff, payer contacts).

Ability to work in compassionate, supportive and collaborative ways with patients and families in an acute medical setting.

Good interpersonal and communication skills with the ability to handle the pressures related to serious life crises and chronic disease situations.
