Responsibilities- Provide one-on-one guidance, support, education, referrals, coordination of care, and other assistance to patients as they move through the healthcare continuum.
- Develops relationships with participating physicians and service providers.
- Liaising with medical providers.
- Guiding patients to appointments
- Coordinating the logistics for plan adherence-appointments
- Supporting patient family members
- High School Diploma or GED equivalent

Skills And Abilities- Ability to be scheduled for work based on operational needs of the hospital.
- Possesses the ability to deal tactfully and harmoniously with guests.
- Demonstrate effective communication skills with all stakeholders (e.g., patients, employer staff, payer contacts).
- Ability to work in compassionate, supportive and collaborative ways with patients and families in an acute medical setting.
- Good interpersonal and communication skills with the ability to handle the pressures related to serious life crises and chronic disease situations.
