Responsibilities
  • Plans, organizes, and implements social work services to patients and families including plans for discharge, aftercare, and linkage to available community resources.
  • Completes crisis assessments for Behavioral Health patients and arranges appropriate disposition.
  • Performs intake assessment/evaluation of clients, including personal, social, and emotional development, family dynamics, and relevant biopsychosocial factors.
  • Participates in multidisciplinary treatment planning with attention to age, gender, sexual orientation, ethnicity, social and cultural influences, and spiritual needs.
  • Assesses patients for discharge planning needs based on decision-making ability, self-care capacity, and placement considerations.
  • Petitions for guardianship/conservatorship for adults who lack DMC where there is no one willing or available to serve as HCS and the patient has no MPOA.
  • Provides documentation of evaluations and recommendations in the medical record; shares interpretations of social, psychological, and emotional problems with the department’s licensed social workers and other team members.
  • Assesses patients for signs or symptoms of abuse and coordinates appropriate referral and counseling; applies knowledge of laws regarding child abuse, neglect, adult abuse, domestic violence, and related topics.
  • Assesses psychosocial needs due to illness causing hospitalization (as age-appropriate) and coordinates care with care managers or physicians.
  • Assists patients and families with end-of-life decision making and advance directives, including counseling on death and dying for various scenarios.
  • Documents services provided with concise entries in the medical record regarding evaluation and plan of care.
  • Evaluates social, financial, psychological, and family situations; refers patients to sources of financial assistance where appropriate.
  • Discusses issues with patients such as advance directives, long-term care, informed consent, and basic patient rights; assists patients in preparing advance directives.
  • Petitions for involuntary commitment for patients within hospital who are deemed a danger to self or others and are refusing treatment.
  • Discusses patient disposition with psychiatrist and treating medical physician; documents disposition accordingly.
  • Creates patient Behavioral Health Treatment Plan based on the patient’s admitting psychiatric problems/diagnosis.
Minimum Qualifications
  • Bachelor’s degree in social work, psychology, sociology or other related field.
  • Obtain Basic Life Support (BLS) Certification within 30 days of date of hire.
  • Complete Crisis Prevention and Intervention (CPI) course or AVADE within 30 days of date of hire.
Preferred Qualifications
  • Master’s degree.
Experience
  • Two (2) years experience in a behavioral health setting.
