We are seeking a qualified Nurse Practitioner to provide patient care in Columbia/Newberry/Irmo and surrounding areas.
OverviewAs a Provider in our physician group, you will deliver exceptional patient care in patients' homes, collaborating with interdisciplinary teams to ensure comprehensive management.
Responsibilities- Evaluate high‑risk patients with complex medical needs and develop individualized care plans.
- Coordinate care activities with team members to ensure continuity of care and optimal patient outcomes.
- Communicate regularly with case managers, clinical coordinators, social workers, specialists, pharmacists, health coaches, and other team members.
- Participate in interdisciplinary team meetings to discuss patient progress and treatment plans.
- Provide care in a variety of settings, including homes, facilities, telehealth, clinics, and specialties.
- Document care activities accurately to support patient care plans and regulatory compliance.
- Educate patients and families on conditions, treatment plans, self‑management, and preventive care.
- Adhere to all relevant healthcare regulations, policies, and procedures, including HIPAA and Medicare guidelines.
- Monitor quality measures and implement corrective actions as needed.
- Valid Nurse Practitioner license in good standing in the state of employment.
- Experience providing patient care in home health, clinic, facility, telehealth, or specialty care settings.
- Strong communication, collaboration, and interpersonal skills within interdisciplinary teams.
- Ability to evaluate high‑risk patients, develop care plans, and coordinate care activities.
- Strong organizational and time‑management skills with ability to multitask and prioritize.
- Basic computer knowledge and ability to maintain electronic health records.
- Licensed driver with reliable automobile in good working order.

Columbia, Newberry, Irmo, and surrounding areas. - Shifts:
12‑hour shift (7 days on, 7 days off) and 8‑hour shift (Monday–Friday, 8:00 am–5:00 pm).
