## Description

** JOB DESCRIPTION**:
• Ability to work as a team member, and agree to supervision by Assistant Director of IDD Services
• Willingness to carry out the mission and support the values of the Emmaus Community
• Supervises team of Residential Advisors (RAs)
• Serves as a Program Specialist for up to 16 individuals and ensures on-going compliance with all 6400 Regulations (especially 44a, 44b (1-19):

• Coordinates and ensures timely completion of assessments     
• Provides assessment for development of ISP, ISP Annual Update & all ISP revisions      
• Participates in the development of the ISP, ISP Annual Update & all ISP revisions      
• Reviews ISP, ISP Annual Update and all ISP revisions & attends all applicable meetings      
• Prepare for and attend SIS & IM4Q meetings      
• Acts as Plan Lead for all individuals receiving services through an SCO      
• Reports content discrepancies to the SC and plan team members      
• Ensures that ISP is implemented as written      
• Supervises, monitors and evaluates services      
• Reviews, signs, and dates the monthly documentation of individuals’ participation and progress toward outcomes      
• Reports changes related to individuals’ needs to the SC and plan team members      
• Reviews the ISP & all 90 Day Reviews with the individual      
• Documents the review of all ISPs every 3 months or when revisions are made      
• Provides documentation of the plan review to the SC and plan team members      
• Informs plan team members of the option to decline the ISP review documentation      
• Recommends revisions that may involve the deletion, addition or modification of an outcome or service      
• Coordinates the provision of services to the individuals      
• Coordinates the training of DSPs in the content of Health and Safety needs relevant to each individual      
• Develops and implements programming to address the acquisition, maintenance, or improvement of functional skills, personal needs, communication and       personal adjustment
• Serves as back up Program Specialist for In Home Service Program Specialist and back up RA as needed
• Provides occasional direct care as needed (ie: in medical emergencies, call offs, etc.)
• Participates in Program Specialist meetings: strategizes with the team on how to resolve household, resident and staff issues;
Responds to staff questions and  concerns
• Carries out incident management responsibilities including entering reports into EIM, data entry corrective action plans, remediation, etc.
• Communicates with all ISP team members as needed including family members, Supports Coordinators, doctors, therapists, behavior specialist, job coaches, day   programs, etc.
• Develops and helps to implement SEE Plans and Restrictive Procedures if applicable
• Assists in staff training on ISPs, SEE plans, Restrictive Procedures, Behavior Plans, Incident Management, Crisis Intervention and other topics related to   programming
• Oversees newly hired DSP shadowing schedules, coordinating between HR and the RA; facilitates smooth transition of DSP scheduling to the assigned RA
• Conducts New Hire Check Ins to ensure smooth transition of DSPs into assigned houses, including technology, scheduling, and training support
• Works minimum of one 5 hour shift per new hire in Team
• Support/ encourage the Team’s DSPs and Senior DSPs going through the Credentialing process
• Meets with Senior Direct Support Professionals for mentoring, training, support, etc.
• Maintains “on-call” responsibilities
