Job Summary And Qualifications

The Mental Health Professional II facilitates high quality, patient‑centered care by providing supervised mental health services to patients and their families. The Mental Health Professional II performs assessments, psycho‑education, process groups, treatment planning, discharge planning and therapeutic counseling support through evidence‑based interventions in individual, family, and group therapy.

Major Responsibilities
  • Conduct individual, group and family psycho educational and process groups.
  • Develop treatment plans and attend treatment planning meetings and reviews on an ongoing basis.
  • Assess environment and patient’s safety status and report/respond to actual and potential safety issues or changes in the patient's condition.
  • Identify risk factors, screening criteria and interventions for patients with special precautions (e.g., suicide, homicide, falls, elopement).
  • Complete assessments, group notes and all other documentation promptly and in compliance with requirements, as appropriate to scope.
  • Collaborate with the multidisciplinary team to assess and coordinate patient’s discharge planning needs and readiness for discharge.
  • Conduct post‑discharge follow‑up as indicated.
  • Evaluate re‑admissions for gaps in discharge planning processes. Formulate a plan and implement new strategies.
  • Develop and maintain a community resource guide including access to public assistance/alternatives.
  • Facilitate and participate in community education, activities and support groups, as appropriate to scope.
  • Contribute to ensuring a safe, structured milieu with evidence‑based treatment modalities/resources.
  • Identify cultural considerations, gender identity and preferred language for patients and family.
  • Demonstrate appropriate application of age/developmental stage specific standards, policies and procedures, and guidelines in assessing and treating patients to ensure patient safety.
