Qualifications

Licensure: LSW, LBSW or LMSW

Experience: Prior work experience in a community or long-term care setting is preferred

Skills: Strong communication, assessment, and social work methods/techniques. Ability to work effectively with multiple disciplines in an individual and team setting. Working understanding of social systems and applicable state and federal regulations. Ability to work in a fast-paced capacity.

Position Highlights

Duties/Responsibilities include, but are not limited to:

  • Complete a social history and psychosocial assessment for each resident identifying social, emotional, and psychological needs.
  • Support individuals and families toward adjustment to center life and cope with illness, disability, and death.
  • Advocate for patients and families in expressing, defining, and resolving grievances.
  • Report to the Director of Social Work Services on the current status of social services and needs.
  • Participate in developing a written interdisciplinary plan of care identifying psychosocial needs and appropriate interventions.
  • Provide therapeutic interventions to aid residents in coping with transition and adjustment to a long-term care facility.
  • Provide support and education to residents and family members about placement and facility issues; refer to social service agencies when necessary.
  • Provide clinical interventions for catastrophic events during a resident's stay.
  • Coordinate discharge planning and refer appropriate home care services before return to the community.
  • Maintain confidentiality of professional information.

Administrative:

  • Develop and maintain a directory of community resources useful to residents and families.
  • Participate in quality assurance interdisciplinary team meetings.
  • Understand and meet government requirements for social service documentation.
  • Document progress in meeting residents’ psychosocial needs.

Advocacy:

  • Work with the interdisciplinary team to promote resident rights and well-being; address abuse as mandated by law and licensure.
  • Identify community changes affecting nursing home residents.
  • Collaborate with residents, families, and staff to support self-advocacy and improve quality of life and care.

Experience

  • Prefer bachelor’s degree from an accredited college or university
  • Knowledge of medical case management, geriatric needs, healthcare settings, and funding resources (Medicare Part A/B/C/D, Medicaid, Managed Care, private insurance)
  • Imagination and creative ability; ability to work with other professionals; organize and carry out responsibilities efficiently
  • Medical knowledge and terminology preferred
  • Marketing experience preferred
  • Ability to interact in person with potential admissions in a hospital setting
  • Multitasking required
  • Commitment to the center’s mission and goals
  • Ability to exercise independent judgment where procedures cannot be standardized
