Position Type: Non‑exempt, full‑time or part‑time, hourly role.
Hours: 2nd shift 3pm‑11pm (every other Monday & Friday and every other weekend).
Pay Class: Part Time.
Rate per Hour: $15.00.

Requirements: Must have an active SC driver’s license for the last 12 months.
- Deliver support per individual service/program plans, including socialization, health maintenance, medication administration, skill/behavioral development.
- Assist with daily activities: personal care, housekeeping, recreational activities, transportation, community integration, shopping, citizenship, and other activities of daily living.
- Provide direct support in residential or periodic programs, typically supporting 4‑6 individuals (residential) or up to 10 (periodic).
- Direct Support:
Assist with socialization, personal care, housekeeping, recreation, transportation, community orientation, shopping, financial management, citizenship, and other activities of daily living. - Records:
Accurately document progress, review records and logs, maintain confidentiality. - Relationships:
Maintain professional relationships with individuals, families, guardians, and case managers; greet visitors; implement Sevita Customer Service Standards. - Reporting:
Report abuse/neglect allegations, medical, behavioral and other incidents in accordance with Sevita policy and external requirements. - Rights:
Respect rights of persons served, practice universal precautions, assist individuals in exercising their rights.
- Accompany individuals to medical appointments; relay orders and information to/from providers.
- Medications:
Accurately administer or support self‑administration, document delivery, report errors, maintain security of controlled medications. - General Health Care:
Monitor health, document concerns, communicate with nurse or supervisor; call 911 in emergencies; follow health directives. - Medical Supplies & Equipment:
Follow procedures for use of equipment; report malfunction or disrepair. - Meal Preparation:
Prepare or support meal preparation per menus and individual choice; monitor mealtimes; implement dining plans.
- Training:
Attend orientation and ongoing training; participate in monthly staff meetings. - Employee Scheduling:
Work scheduled hours; arrive on time; stay until replacement arrives; notify supervisor of absences. - Teamwork:
Work collaboratively, communicate openly, listen, and show respect to others. - Workplace Safety:
Comply with safety policies and procedures; report hazards; participate in safety training.
- Vehicles:
Transport individuals safely; report accidents and safety concerns immediately. - Housekeeping:
Perform daily and seasonal housekeeping; maintain safe environment; report hazards. - Maintenance & Repair:
Monitor safety, perform minor repairs, yard maintenance, and snow removal; report needed repairs. - Safety:
Check water temperature when assisting with bathing; participate in safety drills.
- Perform other duties and activities as required.
- MPA = Mobile Punch Authorized for timekeeping.

- High school diploma or equivalent (preferred).
- Six months of experience in human services (preferred).
- Working knowledge of computers.
- Must be 18 years of age.
- Valid driver’s license in good standing.
- Car registration and vehicle insurance if providing transportation.
- All state‑required training(s) and certification(s) completed in mandated time frames.
- Maintain valid driver’s license, meet insurance requirements, and be approved to drive for the company.
- Work at the program location; may accompany individuals into the community.
- Heavy work: exerting up to 100 pounds of force occasionally, up to 50 pounds frequently, and up to 20 pounds constantly to move objects.
