Burr Hole Evacuation of Subdural Hematoma Craniotomy for Evacuation of subdural hematoma, epidural hematoma, intracerebral hematoma Burr Hole Placement of Ventriculostomy (External Ventricular Drain) Cervical, thoracic, and lumbar non-instrumented spine procedures Estimated Credentialing Timeframe: 60-90 days MANDATED
