Overview
Monitors the medical needs of participants and fulfills nursing requirements as needed; provides personal care and support as needs dictate.
Responsibilities- Monitors overall health status of participants, including but not limited to: regular weighing, blood pressure, etc. Maintains all pertinent records, monitors Therap entries and keeps abreast of all other relevant information. Maintains proper knowledge of service recipients’ health needs.
- Receives and records incoming medications following agency medication procedures; monitors prescriptions and over-the-counter medications; administers medications to participants as needed.
- Monitors general health practices and procedures for all residents.
- Provide general nursing care/treatment to participants as required, including wound dressing and bandaging as well as other special procedures.
- Serve as medical liaison as necessary with family members, professionals and other individuals/organizations in the community when indicated.
- Reviews participant annual physicals.
- Trains staff as needed in the areas of health, proper feeding techniques, lifting, seizures, etc.
- Schedules and accompanies participants to medical/dental appointments or other outings, when necessary.
- Provides overall support offered to residents, including the provision of medical, personal and behavioral supports.
- Provides direct support to residents as needed and role models such supports to DSP staff by assisting with daily living skills, communicating and interacting with residents, facilitating community inclusion, maintaining finances, developing behavioral supports, and demonstrating the importance of residents being active and contributing citizens in their neighborhoods and local community.
- With residents, develops IHP goals and objectives and monitors individual progress.
- Attends trainings as assigned.
- Attends and participates in staff and any other relevant meetings.
- Complies with all agency policies and procedures and any relevant requirements from outside agencies.
- Actively advocates for the rights of residents.
- Cooperates with Agency, DHS and DDD staff in inspections and investigations.
- Performs other duties as assigned.
