Job SummaryAltra Home Health is seeking a compassionate and experienced Licensed Practical Nurse (LPN) to join our team. In this fulfilling role, you will provide individualized, high-quality care to both pediatric and adult patients in their homes. Many of our patients have complex medical needs, including gastrostomy tubes (G-tubes), cerebral palsy, chronic conditions, and, at times, require tracheostomy or ventilator support. You will play a key role in promoting safety, comfort, and quality of life, allowing patients to receive the care they need in the place they feel most at ease—home.
ResponsibilitiesPerform thorough head-to-toe assessments and follow individualized plans of care.
Provide care that may include G-tube feedings, medication administration, wound care, trach care, and ventilator support.
Care for patients with professionalism and empathy.
Educate patients and caregivers on disease management, emergency procedures, and therapeutic techniques.
Collaborate closely with physicians, therapists, and other team members to optimize care.
Document all care activities accurately and promptly in compliance with agency and state requirements.
Maintain rigorous safety, infection control, and privacy standards.
Shift durations range from 4 to 12 hours
, depending on patient needs and nurse availability.Flexible scheduling options available for daytime, evening, and weekend shifts
Opportunities for consistent long‑term cases or variable schedules
Active LPN license in Indiana or an active compact LPN license
At least [1–2] years of clinical nursing experience (home health and pediatric experience strongly preferred)
Experience with G-tube, trach, or vent care preferred (but training will be offered)
Current CPR certification
Strong critical thinking and independent decision‑making abilities
Excellent interpersonal and communication skills
Reliable transportation and a valid driver’s license
