Nurse Practitioner – Diabetic Education (High-Risk Obstetrics) Reports To

Reports to the Practice Manager/Director for administrative supervision and the Practice Site Physician or designee for clinical supervision.

Position Summary

The Nurse Practitioner specializing in Diabetic Education for High‑Risk Obstetric (OB) Patients is responsible for the coordination and delivery of comprehensive primary and specialty care services to BMG patients at designated practice sites. This role focuses on the management, education, and support of pregnant patients with diabetes and other metabolic conditions that place them at high obstetric risk. The Nurse Practitioner works collaboratively with physicians, certified nurse midwives, diabetes educators, nursing staff, and interdisciplinary team members to optimize maternal and fetal outcomes.

The position also provides clinical and administrative support to BMG programs and participates in the education of nursing students, medical residents, and other pre‑professionals.

Essential Duties And Responsibilities Clinical Care Responsibilities

Coordinates and delivers primary, specialty, and nursing care for high‑risk OB patients in collaboration with the Practice Site Physician by:

  • Practicing within the approved scope of practice as designated for a Nurse Practitioner specializing in high‑risk obstetrics and diabetic education.
  • Obtaining comprehensive medical, obstetric, and psychosocial histories, including diabetes history and pregnancy‑related risk factors.
  • Performing complete physical and obstetric assessments and documenting findings accurately.
  • Interpreting and integrating clinical, laboratory, and diagnostic data to develop and implement appropriate diagnostic and therapeutic plans.
  • Managing acute and chronic medical conditions related to pregnancy, including gestational diabetes, pre‑existing diabetes, and associated comorbidities.
  • Ordering and interpreting appropriate laboratory tests and diagnostic procedures.
  • Developing individualized care plans utilizing evidence‑based guidelines with emphasis on prevention, glycemic control, and maternal‑fetal well‑being.
  • Prescribing and administering medications and injections in accordance with prescriptive authority and physician collaboration.
  • Providing intensive diabetic education to patients and families, including glucose monitoring, insulin administration, nutrition counseling reinforcement, lifestyle modification, and pregnancy‑specific management strategies.
  • Educating patients and families on disease processes, medications, treatment plans, and self‑management skills.
  • Triaging patient telephone calls and electronic communications, providing timely clinical guidance and follow‑up.
  • Coordinating care across the continuum, including referrals to maternal‑fetal medicine, endocrinology, nutrition services, and community resources as appropriate.
  • Managing or assisting with medical and obstetric emergencies within the clinic setting.
  • Providing continuity of care between physician visits and throughout pregnancy and postpartum periods.
Programmatic, Education, and Administrative Support

Provides Clinical And Administrative Assistance To BMG Programs By:

  • Participating in patient and group health education initiatives related to diabetes and high‑risk pregnancy.
  • Maintaining accurate clinical documentation, registries, and required statistical data.
  • Preparing reports and tracking outcomes related to diabetic education and high‑risk OB care.
  • Participating in interdisciplinary care conferences, case reviews, and care coordination meetings.
  • Assisting with referrals and communication with internal and external providers and vendors.
  • Contributing to the development, implementation, and evaluation of ongoing education and follow‑up programs designed to measure patient outcomes.
