Oak Street Health is hiring a Medical Scribe (Clinical Informatics Specialist) to support our primary care providers with clinical documentation. The role focuses on real‑time documentation, support of the value‑based care model, and population health tools.

• Join the provider in the exam room to observe patient visits.
• Document patient encounters in a structured note (history of present illness, assessment, plan, physical exam).
• Assign appropriate CPT and ICD‑10 codes.
• Prepare after‑visit summaries.
• Consult with the provider to ensure accurate and specific documentation.
• Clinical Documentation Improvement (≈10%)
  • Request and review medical records.
  • Leverage Oak Street’s population health tools to support improvement.
  • Prepare for and support daily huddles and clinical documentation reviews.
  • Consult with provider on documentation opportunities.
• Administrative support for the provider and care team (≈10%)
  • Place orders and referrals.
  • Address related tasks.
  • Support the care team with documentation responsibilities.
• Other duties as assigned.

Knowledge

• Medical terminology and common medications (pre‑medical degree or prior clinical experience) – required.
• Prior clinical experience (shadowing/volunteering) – strongly preferred.
• Prior scribe or transcription experience – preferred.

Skills

• Advanced listening and communication – required.
• Strong computer literacy and ability to learn new technical workflows – required.
• Fluency in Spanish, Polish, Russian, or other community languages – required where indicated.

Abilities

• Adapt to new workflows quickly – required.
• Typing 70+ words per minute – strongly preferred.
• Take direction and work as part of a team providing patient care – required.
• Be a self‑starter within role scope.
• Excellent job attendance and ability to work in‑person in our clinics – required.
• Commit to at least 1 year in the role (2+ ideal) – required.
• Work approximately 40‑45 hours per week during clinic hours – required.
• Compliance with hospital and Oak Street Health policies, including HIPAA – required.
• US work authorization – required.

: 40

Pay Range
: $17.00 – $25.65 per hour (base)
