Position: Assistant Team Lead (Mental Health Services)

Overview

The ACCS Assistant Team Leader serves as a primary resource for management, clinical expertise and staff development of an Adult Community Clinical Services (ACCS) Team. The ACCS Teams are interdisciplinary teams of outreach workers designed to provide comprehensive clinical and rehabilitative services to adults with psychiatric conditions. Working under the ACCS Team Leader, the ACCS Assistant Team Leader monitors engagement of all Persons Served (PS), selects intervention strategies for those who are difficult to engage, and designs strategies which will ultimately reduce barriers to living independently in their communities.

The ACCS Assistant Team Leader will train, mentor and model clinical evidence-based practices to all team members and to staff based in Group Living Environments and Supported Independent Environments (GLE/SIE).

Responsibilities
  • Assist in the oversight of clinical programming and service delivery by the team to ensure the needs of Persons Served are adequately met.
  • Provide clinical supervision to clinically licensed and other assigned staff.
  • Provide competency-based staff training on clinical evidence-based practices to ACCS team members and GLE/SIE Program Directors.
  • Consult with Licensed staff on appropriate clinical interventions for treatment plans, care transitions, and crisis prevention tactics.
  • Function as a primary liaison with the Department of Mental Health.
  • Perform other related duties as assigned.
Additional Responsibilities
  • Oversee clinical programming and service delivery by the team.
  • Monitor engagement status of all PS, consult on intervention strategies, and support LPHAs in effectively addressing lack of initial or sustained engagement for any PS.
  • Collaborate in orientation, engagement and enrollment activities for PS presenting high risk behaviors.
  • Provide competency-based staff training on clinical evidence-based practices and best practices to team members and GLE/SIE Program Directors, including Motivational Interviewing;
    Harm Reduction and Addiction Treatment; SBIRT;
    Housing First;
    Stages of Change;
    Seeking Safety;
    Cognitive Behavioral interventions;
    Safety Plans and Crisis Prevention/Response Plans; 8 Dimensions of Wellness; and documentation standards.
  • Mentor and model best practices and evidence-based interventions for all staff.
  • Support staff use of trauma-informed, recovery-oriented, person-centered, culturally competent and strengths-based approaches and skills training techniques.
  • Support LPHA and Licensed staff performance with supervision, feedback, competency assessments, and field supervision to observe interactions with PS.
  • Promote healthcare, employment and education service access for PS.
  • Promote engagement, housing permanency, community tenure, self-sufficiency and successful completion of services.
More Specific Responsibilities
  • Consult with LPHAs and Licensed staff on interventions for treatment plans for difficult-to-engage PS.
  • Review LPHA treatment plans to ensure effectiveness and timely documentation.
  • Oversee clinical risk assessment and management strategies; meet with PS as needed for assessments and program participation.
  • Consult with LPHAs to complete Safety Plans and Crisis Prevention/Response Plans.
  • Lead risk management and crisis prevention efforts for high-risk PS; train staff on risk management.
  • On-site consultation to address engagement, risk and other clinical needs; ensure integration of risk management processes.
  • Manage challenging care transitions with LPHAs;
    Guardian-related activities and liaison with DMH as needed.
  • Notify DMH of significant events; manage adverse events and corrective actions; request risk management support as needed.
  • Ensure adherence to Vinfen risk mitigation protocols and analyze risk data for practice improvements.
  • Convene Treatment Planning meetings; coordinate with Integrated Team Consulting Psychologist and Occupational Therapist; seek senior leadership consultation as needed.
  • Cover Integrated Team Leader duties during absences and may perform LPHA coverage when LPHA staff are absent.
  • Participate in internal communities of practice and committees as assigned.
  • Perform other related duties as assigned.
