About Company:

Blue Summit Hospice and Palliative Care was founded based on the knowledge that great clinical care can only be achieved by a great clinical staff. Our company was started by clinicians with hands-on experience working in end-of-life care. And our goal is not only to provide the best possible care but also to be the best possible place to work.

We place as much focus on staff training and retention as we do on our patient care. And that's how it should be! End-of requires a sincere commitment to meeting the unique needs of the patients and families we serve. Blue Summit has that same level of commitment to our staff.

If you are a clinical end-of-life professional or simply want more information about our services, reach out to us, and we'll be happy to explain the Blue Summit difference.

About the Role:

The Palliative Care Nurse Practitioner (NP) for Home Visits plays a critical role in delivering compassionate, patient-centered care to individuals with serious, chronic, or life-limiting illnesses in their own homes. This part-time position, requiring availability three days per week, focuses on managing symptoms, coordinating care, and improving quality of life for patients and their families. The NP will collaborate closely with interdisciplinary teams, including physicians, social workers, and hospice providers, to develop and implement individualized care plans.

By providing expert clinical assessment, pain and symptom management, and emotional support, the NP ensures that patients receive holistic care tailored to their unique needs. Ultimately, this role aims to enhance patient comfort, dignity, and autonomy while supporting families through complex healthcare decisions in the home setting.

Minimum Qualifications:

  • Current and unrestricted Nurse Practitioner license in the United States.
  • Certification as a Nurse Practitioner with a focus on Adult-Gerontology or Family Practice.
  • Minimum of 2 years clinical experience in palliative care, hospice, or related nursing specialty.
  • Basic Life Support (BLS) and Advanced Cardiovascular Life Support (ACLS) certifications.
  • Ability to travel to patient homes within the assigned geographic area.

Preferred Qualifications:

  • Certification in Hospice and Palliative Nursing (CHPN) or equivalent specialty certification.
  • Experience working in home health or community-based care settings.
  • Familiarity with electronic health record (EHR) systems and telehealth technologies.
  • Strong background in pain and symptom management for complex chronic illnesses.
  • Training in communication skills specific to end-of-life care and advanced care planning.

Responsibilities:

  • Conduct comprehensive patient assessments during home visits to evaluate physical, emotional, and psychosocial needs.
  • Develop, implement, and adjust individualized palliative care plans in collaboration with patients, families, and healthcare teams.
  • Manage symptoms such as pain, nausea, fatigue, and breathlessness using evidence-based interventions and pharmacologic treatments.
  • Provide education and counseling to patients and caregivers regarding disease progression, treatment options, and advance care planning.
  • Coordinate care with primary care providers, specialists, hospice services, and community resources to ensure seamless support.
  • Document all patient interactions, assessments, and care plans accurately and timely in the electronic health record system.
  • Respond promptly to patient and family concerns, providing crisis intervention and support as needed.
  • Participate in interdisciplinary team meetings to review patient status and update care strategies.

Skills:

The Palliative Care Nurse Practitioner utilizes advanced clinical assessment skills daily to evaluate and manage complex symptoms in homebound patients. Effective communication skills are essential for educating patients and families, facilitating difficult conversations about prognosis and care preferences, and coordinating with interdisciplinary teams. Time management and organizational skills enable the NP to efficiently plan and conduct home visits while maintaining thorough documentation.

Critical thinking and problem-solving abilities are applied to tailor care plans that address evolving patient needs and unexpected clinical challenges. Additionally, proficiency with electronic health records and telehealth tools supports accurate record-keeping and remote patient monitoring, enhancing continuity of care.
