Overview

We are seeking a Medical Social Worker (MSW) for a part-time role in Smyrna, TN, supporting home health patients in surrounding communities.

• Provide emotional support, counseling, and resource coordination to meet patients' psychosocial needs with dignity.
• Assist patients and families with social, emotional, and financial concerns, promoting effective coping skills.
• Communicate clinical changes to the healthcare team and educate on social work interventions.
• Perform care planning, discharge, and risk assessment to ensure safe, holistic care.

• Must hold an MSW from an accredited school
• Must hold a valid state license
• One year of healthcare social work experience
• Reliable transportation
