OverviewA staff Physical Therapist will follow appropriate practices, to perform evaluations, timely re‑evaluations and OASIS assessments, determine PT diagnoses, develop a plan of care, and perform physical therapy procedures to aid in restoring function, improving mobility, relieving pain, improving health, and preventing or limiting permanent physical disability. A staff Physical Therapist must demonstrate the knowledge and skills necessary to provide age, goal, and diagnosis appropriate care.
This position must also be able to follow and adhere to the practices described in the Department Policy and Procedure Manual. The Therapist is required to be flexible with daily scheduling.
- Perform evaluations, determine PT diagnoses, develop plan of care, and perform physical therapy procedures to aid in restoring function, improving mobility, relieving pain, improving health, and preventing or limiting permanent physical disability.
- Evaluate and assess the patient timely, based on principles outlined by practice guidelines, evidence‑based guidelines and regulatory standards (state & organizational policies, requirements of accrediting bodies, etc.).
- Provide reassessments including OASIS assessments at appropriate time points to determine the level of functioning; update and revise the plan of care in consultation with the physician and interdisciplinary team members.
- Build trusting collaborative relationships with a multi-disciplinary team to support patient care needs; actively participate in multi-disciplinary team meetings, as assigned.
- Supervise Physical Therapy Assistants and Home Health Aides in providing services, including completion of required supervisory visit documentation. Participate in training and support of all team members, volunteers and students.
- Provide therapeutic treatments and evaluate equipment needs as applicable; prepare clinical and progress notes that support skilled intervention and assess outcomes of care; plan for discharge of patient.
- Carry out treatment plan effectively and modify it to maximize therapeutic benefit and meet discharge criteria and return to optimal level of care. Utilize testing results and develop updated plan of care based on principles outlined in appropriate evidence‑based guidelines.
- Bachelor’s Degree, Physical Therapy (Required)
- Master’s Degree, Physical Therapy (Required)
- Doctoral Degree, Physical Therapy (Required)
- 1 year Physical Therapy experience (Preferred)
- Demonstrates knowledge of the principles of growth and development throughout the life span, believes able to assess patients’ needs and status by analyzing evaluated data, and be able to interpret information to identify age‑specific needs and provide appropriate care accordingly per patient care setting.
- License Physical Therapy (PT) – Virginia Department of Health Professions (VDHP) (Required)
- CPR/BLS Certification – American Heart Association/American Red Cross/American Safety and Health Institute (AHA/ARC) within 30 Days (Required)
- Valid Driver’s License (Required); drives personal vehicle for RHS business 25% or more of the time to perform essential functions of the job
