Key Responsibilities:

As a Registered Nurse, your responsibilities will include:

  • Performing initial assessments and actively participating in the development and maintenance of treatment and care plans.
  • Taking part in weekly on-call rotation.
  • Initiating and updating the physician/practitioner's treatment plan.
  • Developing, implementing, updating, and effectively communicating patient care plans.
  • Conducting re-certification and discharge assessments as appropriate.
  • Implementing current nursing practices during all home health care visits.
  • Assessing patient conditions and needs during each visit.
  • Providing skilled nursing services, including treatments, diagnostics, preventative measures, and rehabilitative procedures based on patient needs.
  • Maintaining accurate and timely clinical notes that are incorporated into the clinical record.
  • Assisting in the coordination of all home care services.
  • Conducting supervisory visits to evaluate the performance of Agency home health aides in skilled patient care.
  • Utilizing equipment and supplies safely and efficiently.
Qualifications:

We are looking for candidates who have:

  • A current and unencumbered RN license in Louisiana.
  • CPR/BLS Certification.
  • Proven competence to provide care independently, adhering to nursing practice standards.
  • The ability to work both independently and collaboratively in a team environment.
  • Experience with Electronic Medical Records (EMR).
