Overview
We are seeking a compassionate and dedicated Caregiver to provide exceptional end-of-life care for patients and their families in our hospice house setting. This role combines direct patient care, medication administration, light household support, and emotional guidance during one of life's most sacred transitions. The ideal candidate will demonstrate clinical-like competency alongside deep empathy, ensuring both patients and families feel supported, dignified, and comforted.
Essential Duties and Responsibilities- Provide compassionate, person-centered care that honors each patients individual needs, preferences, and dignity
- Monitor and document patient condition, comfort levels, and any changes in status ensuring patients are kept clean and dry
- Collaborate with hospice nurses and interdisciplinary team members to ensure comprehensive care
- Maintain patient safety and implement fall prevention measures
- Provide comfort measures and pain management support as directed by nursing staff
- Administer medications accurately according to hospice nurse orders and established protocols
- Document medication administration and monitor for side effects or adverse reactions
- Maintain proper medication storage, security, and inventory
- Follow all regulatory requirements for medication handling and documentation
- Prepare meals using family-provided ingredients, accommodating dietary restrictions and patient preferences
- Maintain clean, safe, and comfortable living environment through light housekeeping duties as noted in cleaning binder
- Perform laundry services for patient linens and personal items
- Organize patient living spaces to promote comfort and accessibility
- Provide emotional support to family members during the caregiving process
- Facilitate meaningful moments between patients and loved ones
- Demonstrate cultural sensitivity and respect for diverse family dynamics and traditions
- Offer bereavement support and resources to families as appropriate via the hospice agency
- Maintain accurate, timely documentation of all care provided
- Follow HIPAA regulations and maintain strict confidentiality
- Adhere to hospice policies, procedures, and regulatory requirements
- Participate in care plan meetings and quality improvement initiatives
- Complete required continuing education and training programs
- Hospice or palliative care training preferred
- Must NOT have a CNA License
- Minimum 2 years experience in healthcare setting, preferably in hospice, long-term care, or home health
- Experience working with terminally ill patients and bereaved families preferred
- Demonstrated experience in medication administration and documentation
- Exceptional communication and interpersonal skills
- Strong assessment and observation abilities
- Ability to work independently while maintaining team collaboration
- Cultural competency and sensitivity to diverse populations
- Physical ability to lift up to 50 pounds and assist with patient mobility
- Emotional resilience and ability to cope with grief and loss
- Deep compassion and empathy for patients and families facing end-of-life transitions
- Patience, gentleness, and respect for human dignity
- Emotional maturity and stability in challenging situations
- Strong ethical foundation and professional integrity

- Variable shifts including days, evenings, weekends, and holidays as patient needs require

- Exposure to infectious diseases and medical equipment
- Physical demands include prolonged standing, bending, and lifting
