Position Summary
We are seeking a compassionate and skilled PRN Social Worker to provide psychosocial support and care coordination services on an as-needed basis. This role focuses on assessing patient and family needs, connecting individuals with appropriate resources, and supporting safe, effective transitions of care.
Key ResponsibilitiesConduct psychosocial assessments to identify patient, family, and social support needs
Provide counseling, emotional support, and crisis intervention within scope of practice
Assist with discharge planning, care coordination, and referrals to community resources
Advocate for patients and families to ensure access to appropriate services
Collaborate with physicians, nurses, therapists, and interdisciplinary team members
Document assessments, interventions, and care plans accurately and in a timely manner
Ensure compliance with ethical standards, regulatory requirements, and organizational policies
Participate in care conferences and interdisciplinary meetings as needed
Bachelor's or Master's degree in Social Work (BSW or MSW) from an accredited program
Current state Social Work license or certification in good standing (as required by role)
Knowledge of community resources, healthcare systems, and discharge planning processes
Strong communication, organizational, and problem-solving skills
Ability to work independently and manage a variable caseload
Prior experience in a healthcare or clinical setting (e.g., hospital, rehabilitation, SNF, home health)
Experience with electronic medical records (EMR) systems
Case management or care coordination experience
Flexible availability, including weekdays, weekends, or holidays as needed

Hours vary based on patient census and organizational needs
Ability to sit, stand, and move throughout clinical areas as needed
Emotional resilience and ability to manage high-stress or sensitive situations
Healthcare setting may include hospital, outpatient clinic, rehabilitation facility, skilled nursing facility, or community-based locations (depending on role)
