Job Overview
Airedale Cardiac Rehabilitation Service aims to support people and their families following recent heart attacks or expected cardiac interventions such as stents or internal defibrillators. We also provide support for people prior to and following heart surgery.
Our team comprises of Cardiac Rehab Specialist Nurses, a Healthcare Support Worker and administration support, we work alongside the wider community nurse specialist team. We are also supported by the therapy services to run our cardiac rehab exercise classes and have access to several of the multi‑disciplinary teams.
A cardiac rehab specialist nurse has an amazing opportunity to make a big difference in a patient’s life in such a positive way that it is a true privilege.
An essential but very rewarding aspect of the role is to quickly gain rapport with a patient and their families so that we can promote self‑care and offer individualised care. This means we are fortunate in being able to offer patients the time they need which can be a rare commodity in the current NHS.
Main duties of the jobThe role is varied and interesting, we offer flexibility to our patients and our employees as much as possible with patient care being offered in their own homes, clinics, community venues, video calls and the hospital.
We are a small friendly focused team who are passionate about promoting self‑care to empower our patients to keep their hearts healthy. We have developed our service to include virtual options alongside our face to face activities.

Person specificationEssential criteria
- Current UK Driving Licence and access to own vehicle for business use
- Ability to travel across a large geographical area
Essential criteria
- Qualified nurse registered with NMC
- Recognised qualification in Teaching and Assessing
- Successful completion of a professionally validated specialist courses.
- Leadership Courses
Essential criteria
- Two years relevant experience working at Band 5 level
- Relevant and recent experience working in cardiology
- Experience of multi‑disciplinary team working and working collaboratively with other professions and agencies including statutory, private, and voluntary sector services.
- Experience of cardiac rehab assessments and treatments
- Experience of working within primary care and community settings
- Experience of taking part in or leading on projects/service improvements
- Experience of providing personalised care and promoting self‑care
Essential Criteria
- Knowledge of Cardiac Rehabilitation
- Evidence of continuous professional development
- Understanding of the NHS Constitution and its implications.

- Knowledge of cardiac rehab patient management in acute or community setting.
