The post holder will be responsible for ensuring the highest standard of clinical care is provided for patients referred to the team for District Nursing Services which is delivered over a 24 hour period.
In pursuing these duties the post holder will ensure compliance with the NHS Constitution
1 x Full Time post (37.5 hours)
1 x Part Time post (22.5 hours)
Main duties of the jobThe NHS Plan sets a challenging agenda of improvement, expansion and reform for health and social care providers. This includes devolution of power and resources to the front line to give health professionals who deliver care the freedom to innovate. The competencies described are based on the NatPACT PCT Competency Framework for Nurses, Midwives and Health Visitors (2003). The post holder will support the Team Leader in the provision of strong clinical leadership and in ensuring the principles of clinical governance are implemented at a local level within the designated team.
The post holder must adhere at all times to the Code of Conduct as published by the Nursing and Midwifery Council and must work within the policies and guidelines as laid down by Airedale NHS Foundation Trust.

Person Specification Knowledge- Understanding of the NHS Constitution and its implications
- Registered Nurse
- Significant experience and competence in community nursing
- Able to assess patients with complex health care needs in community settings and develop care plans to meet needs.
- Experience of risk management and governance.
- Knowledge/experience of working in a multi-cultural community
- Teaching & Assessment Qualification (ie ENB 998)
- Commitment to clinical supervision
- Extended/Supplementary Nurse Prescriber
- Blurred boundary working
- Care and Management of PICC and Hickman Lines
- IV Therapy Administration
- Verification of death
- Ability to maintain accurate, timely and contemporaneous records
- Excellent interpersonal skills
- Experience of Systm One use
- IT skills (use of email, internet, word processing, etc)
- Ability to input, amend and modify data.
- Understanding confidentiality and Data Protection.
- Supervise and assess staff performance and development.
- Access clinical supervision
- Ability to be flexible to cover the needs of the service, including weekends/bank holidays, where appropriate.
