RN - Case Management

The Registered Nurse will be responsible for the active management of patients, collaborating with medical providers and interdisciplinary team members to establish individualized medical goals that maximize patient healthcare outcomes. The RN will address patient needs promptly and efficiently while ensuring proper coordination of referrals to Surgery/Specialty Clinics.

• Collaborate with medical providers to set personalized healthcare goals.
• Manage patient referrals and track progress of care.
• Serve as the primary clinical contact for care coordination.
• Perform nursing duties including assessment, documentation, and medication administration.
• Maintain annual competencies in safety and infection control.
• Participate in team and departmental improvement efforts.

• Graduate of an accredited school of nursing (Bachelor’s degree preferred).
• At least 1 year of case management experience.
• Active RN license in the State of Alaska.
• Basic Life Support certification.
