• Assess resident health and monitor for changes.
• Develop and update personalized care plans in collaboration with residents, families, and the care team.
• Administer medications and treatments safely and accurately.
• Observe residents for medication reactions or changes and report concerns.
• Provide wound care, IV therapy, catheter management, and other skilled procedures within scope of practice.
• Supervise and support LPNs and CNAs, delegating appropriately.
• Educate residents and families on medications, treatments, and chronic condition management.
• Document care provided and resident progress in a timely manner.
• Respond quickly to emergencies, delivering critical care and coordinating with medical teams.
• Work closely with physicians, therapists, and other professionals to ensure coordinated care.
• Perform other duties as assigned within the scope of practice.
• Adhere to facility safety protocols, infection control guidelines, and regulatory standards.

• One year of recent experience as an RN in a long‑term care setting.
• Valid RN license in compliance with state regulations.
• Current BLS (AHA/ARC) or CPR certification.

• ACLS Certification.
• Other certifications may be required depending on facility requirements.

• Critical thinking, service excellence, and good interpersonal communication skills.
• English language reading, writing, and communication proficiency.
• PC skills for documentation and communication.
• Physical abilities: remain stationary, move equipment (50‑100 lbs), push, pull, bend.
• Interpersonal skills: work effectively with diverse personnel, maintain professionalism.
• Technical/motor skills: fine manipulation, assist with procedures, use department equipment.
• Mental requirements: manage frequent public contact, meet deadlines under pressure, handle irregular activity schedules, maintain concentration to detail.
• Sensory: visual acuity and effective communication ability.
