- Assess resident health and monitor for changes.
- Develop and update personalized care plans in collaboration with residents, families, and the care team.
- Administer medications and treatments safely and accurately.
- Observe residents for medication reactions or changes and report concerns.
- Provide wound care, IV therapy, catheter management, and other skilled procedures within scope of practice.
- Supervise and support LPNs and CNAs, delegating appropriately.
- Educate residents and families on medications, treatments, and chronic condition management.
- Document care provided and resident progress in a timely manner.
- Respond quickly to emergencies, delivering critical care and coordinating with medical teams.
- Work closely with physicians, therapists, and other professionals to ensure coordinated care.
- Perform other duties as assigned within the scope of practice.
- Adhere to facility safety protocols, infection control guidelines, and regulatory standards.
- One year of recent experience as an RN in a long‑term care setting.
- Valid RN license in compliance with state regulations.
- Current BLS (AHA/ARC) or CPR certification.
- ACLS Certification.
- Other certifications may be required depending on facility requirements.
- Critical thinking, service excellence, and good interpersonal communication skills.
- English language reading, writing, and communication proficiency.
- PC skills for documentation and communication.
- Physical abilities: remain stationary, move equipment (50‑100 lbs), push, pull, bend.
- Interpersonal skills: work effectively with diverse personnel, maintain professionalism.
- Technical/motor skills: fine manipulation, assist with procedures, use department equipment.
- Mental requirements: manage frequent public contact, meet deadlines under pressure, handle irregular activity schedules, maintain concentration to detail.
- Sensory: visual acuity and effective communication ability.
