Registered Nurse - Community Living Center

This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. You must meet specific eligibility requirements per VHA policy and submit your EDRP application within four months of appointment. Program Approval, award amount (up to $200,000) & eligibility period (one to five years) are determined by the VHA Education Loan Repayment Services program office after review of the EDRP application.

Rehabilitation and Extended Care (REC) Registered Nurse (RN) is responsible for all elements of the nursing process when providing and supervising direct patient care. Assesses, plans, implements, and evaluates care based on age-specific components. Coordinates patient care focused on transition through the continuum of care, patient and family education, and self-management after discharge. Administers medications and performs procedures per established policies.

* Work with the care team to sign off and ensure completion of all orders written for assignment and Licensed Practical Nurse (LPN) teammates

* Administer medications and perform procedures per established policies and guidelines

* Conduct daily and weekly skin assessments, monitor and document pain and pain interventions

* Check crash carts for readiness and respond to codes as part of the code team

* Collaborate with members of the interdisciplinary team to influence care outcomes

* Participate in core elements of performance including knowledge and active participation in unit or program level quality improvement

* Establish therapeutic relationships with patients, families, and caregivers to incorporate knowledge, values, and beliefs into care planning

* Apply evidence-based practice and research to patient care and participate in unit-based activities to improve and deliver cost-effective care

Pay:
