** Ava’s House is a 20-bed inpatient acute hospice facility. We care for adult and pediatric patients and have cared for patients ranging from the age of one day old to over 100 years old. We offer a collaborative interdisciplinary team that promotes engagement and education. Our interdisciplinary team is made up of physicians, nurse practitioners, nurses, patient care techs, social workers, pharmacists, volunteer representatives, and chaplains.

Our facility focuses on symptom management and maintaining comfort and dignity at end of life. We provide 24-hour care and could be considered a “Hospice ICU”.
We take deep pride in our ability to manage distressing symptoms in medically and ethically appropriate ways. We understand that this is a very difficult time for our patients and their families, and we work tirelessly to meet them where they are on their journey. Our department works closely with all inpatient units, especially medical oncology. Medical oncology staff care for our patients until they are able to be transferred to Ava's House.
** Summary
* * The Registered Nurse in the Clinical Care Leader (CCL) role facilitates health care services for patients in cooperation with the health care team. Facilitates communication and provision of information and messages in a way that produces clarity and impact. Understands the importance of inter-team collaboration in breaking down silos and achieving business results and the ability to lead employees from various functions to communicate, coordinate work across divisions, and collaborate in solving problems as one team.

Applies the concept of collaboration to build relationships and coordinate with other teams. Provides feedback to members of the health care team.
** Job Description
** Responsible for utilizing the nursing process (assessment, diagnosis, outcomes/planning, implementation and evaluation) to provide individualized nursing care to patients. Collaborates with other inter-professional colleagues, including physicians, to plan, implement and evaluate care. Demonstrates competency and practices within the full scope of nursing expertise/knowledge and utilizes appropriate age and population specific standards as designated in their assigned clinical setting. Cares for patients in all phases of preventative care, health maintenance, diagnosis, treatment, and follow-up as patients move along the continuum of care.

Responsible for the coordination of care, patient assessment, patient education, triage, and various other nursing interventions. Functions within the scopes and standards of nursing practice as outlined in the Nurse Practice Act and Administrative Rules in state of practice and licensure. The Sanford Professional Nursing Practice recognizes the Scope and Standards of Practice and the Code of Ethics for Nurses with Interpretive Statements as published by the American Nurses Association as the foundation of nursing care delivery and professional conduct.

Competent in the registered nurse (RN) skills of the unit/department. Knowledge of procedures and programs to ensure employee safety. Assumes leadership role in high-risk situations. Coordinates the clinical/unit nursing activities while adhering to Sanford Health philosophy, goals, and standards of care.
