Job Summary

The Nursing Assistant (NA) serves as caregiver to the resident during the scheduled work period in long term care. Provides resident-centered nursing care and daily living assistance to assigned resident under the supervision of a registered nurse (RN). Delivers age-appropriate care related to the physical and psychological needs of the resident as per care plan. Maintains infection prevention and control policies and uses Personal Protective Equipment as required.

Provides assistance with basic health care needs including daily living activities such as bathing, toileting, grooming, dressing/undressing, obtaining and recording vital signs, and providing psychosocial support and other personal care to the resident. Assists with transferring, repositioning, and ambulation using correct techniques and equipment, and provides range of motion and passive exercises. Supports dining activities and prepares a positive dining experience for residents.

Assists residents with meals and snacks, provides water and supplements as appropriate.

The NA aids providers and nursing staff with procedures, documents resident interactions and outcomes related to care, communicates changes in condition to the charge nurse, and supports wellness and recreational programs. May participate in departmental/organization committees as needed.

Responsibilities
  • Serve as caregiver to residents during scheduled shifts in long-term care.
  • Provide resident-centered nursing care and daily living assistance under RN supervision.
  • Follow care plans and infection prevention/control policies; use PPE as required.
  • Assist with activities of daily living, vital signs, and psychosocial support.
  • Assist with transferring, repositioning, gait, and transfers using proper techniques and equipment.
  • Support dining, meal assistance, and hydration for residents.
  • Document resident interactions and outcomes; communicate changes to the charge nurse.
  • Participate in wellness and recreational activities and contribute to a positive resident experience.
Qualifications
  • High school graduate or GED preferred, but not required.
  • Minimum age of 16. Prior clinical or nursing assistant experience in long-term care preferred.
  • Required Certified Nursing Assistant (CNA) certification with the State Board of Nursing, or state certified CNA within four months of date into the position by being enrolled in a training program or in the process of obtaining certification through Endorsement. CNA training and certification required and provided by facility.
  • Satisfactory completion of yearly department-specific competency testing and maintenance of continuing education credits (minimum 12 hours annually). Maintain all department-specific required certifications.
  • If applicable to state, certified and registered with the Unlicensed Assistive Person (UAP) Registry.
  • Role may classify as a Sanford Category II Driver depending on facility requirements; may require motor vehicle record and valid driver’s license and medical qualification to drive as per policy.
