Position Summary

Provides home health nursing care and health teaching to adults in agency service area in accordance with agency policies and procedures. Oversees and manages the plan of care, physician orders for treatments and services of patients as assigned. Maintains professional development. Performs on call and after hours duties as assigned to meet need of the agency.

Job Functions
  • Utilizes sound clinical judgment and appropriate assessment skills in the provision of nursing services for home care. Reports abnormal findings or additional needs to the physician in charge of patient’s care in a timely manner. Makes referrals to other disciplines or services when ordered.
  • Maintains awareness of McLean continuum programs and services such as Lifeline, Meals on Wheels, Adult Day Care, etc., and makes referrals as appropriate an desired by patient/family.
  • Adheres to infection control and exposure prevention practices as outlined by agency policies and procedures. Maintains appropriate supplies and demonstrates bag technique during home visits.
  • Develops and documents the plan of care upon admission to service for each patient. Documents assessment and care provided thoroughly and accurately. Completes and submits required documentation in a timely manner and in accordance with agency policy and clinical protocols. Documents revisions to the plan of care as goals are met and/or additional interventions are identified and ordered.
  • Provides nursing care in accordance with plan of care as well as agency policy and procedure and accepted standards of practice. Assesses learning needs of patients and families and utilizes effective verbal and/or written methods to achieve learning goals.
  • Implements the physician ordered plan of care using available resources with awareness of payment source and agency practice guidelines. Works with agency personnel to obtain authorization for all visits, supplies prior to providing to patients.
  • Continually evaluates effectiveness of the patient’s plan of care for skilled and home health aide services. Revises plan of care at least every 60 days and as necessary. Regularly evaluates the patient’s progress towards goals and initials discharge planning and termination of services when appropriate. Communicates discharge plans to patient and family as appropriate.
  • Coordinates care and documents case conferences with other disciplines, multidisciplinary team meeting notes in accordance with agency standards.
  • Obtains verbal order for continuation of agency services when ongoing skilled need remains. Completes re-certification assessments and documents modifications to the plan of care in accordance with regulation and agency policy.
  • Evaluates safety and appropriateness for delegation of tasks/duties when establishing plan of care for home health aide services. Provides instruction and ongoing supervision to home health aides under established HHA plan of care for patients on caseload or within territory in accordance with state licensure regulations and agency policy. Delegates initial instruction of HHA to therapist when appropriate, for cases with no nursing services ordered.
  • Provides feedback on home health aide performance to the HHA Supervisor and the Quality and Reimbursement Supervisor.
  • Participates in agency Quality Improvement Program. Identifies areas for improvement within agency processes as well as personal/professional learning needs. Regularly attends staff meetings, in‑services and meets competency and education requirements of position. Participates in the orientation and training of nursing staff as assigned.
