Home Health Licensed Vocational Nurse; LVN - Simi Valley

, headquartered in San Ramon, CA, is currently seeking a Licensed Vocational Nurse (LVN) to join our Home Health Care Agency located in Northridge, CA. The Licensed Vocational Nurse is responsible for providing direct patient care under the supervision of a registered nurse. Responsibilities include following the plan of care, providing treatments, and working collaboratively with the members of the team to help meet positive patient care outcomes. A licensed vocational nurse shall perform duties consistent with the Vocational Nursing Practice Act. A licensed vocational nurse shall meet qualifications established by the home health agency for the services provided and any additional qualifications required by home health agency licensure regulations.

Per Diem (12 visits per week). Non-traditional schedules are available, i.e., Thursday-Monday.

Area #45 covering the cities of Agoura Hills, Calabasas, Moorpark, Newbury Park, Oak Park, Simi Valley, Thousand Oaks and Westlake Village;
Area #42 covering the cities of Canoga Park, Chatsworth, Encino, Lake Balboa, Reseda, Tarzana, West Hills, Winnetka, Woodland Hills.

Mon-Fri 9am - 5:30pm onsite at our Woodland Hills office.

• Minimum one year of LVN experience required
• Must possess a CA License with IV/Blood Withdrawal Certification
• Reliable Transportation and auto insurance
• CPR Certification

This is NOT a remote position
