Job Description:

Provide essential services to patients and families incidental to hospitalization. Responsibilities include counseling, support, and developing transition plans. The role involves addressing non-medical problems and coordinating post-discharge planning with outside agencies, nursing staff, and relatives.

Requirements & Qualifications:

• Licensure: Active Social Work license for California.
• Education: Master’s Degree in Social Work (MSW) – Required in CA.
• Experience: Minimum 1 year in an acute or long-term medical setting (Required). 2+ years preferred.
• Specific Experience: Must have acute care, ED, and experience working with the homeless population.
