Responsibilities:
- Manage a caseload of 15-20 hospice patients, delivering individualized nursing care in their homes.
- Collaborate with an interdisciplinary team and participate in weekly case meetings to ensure cohesive care delivery.
- Assess, plan, and update patient care plans, applying nursing judgment based on ongoing evaluation and patient goals.
- Coordinate care with physicians, hospice medical directors, and other disciplines to provide seamless treatment and symptom management.
- Respond promptly to changes in patient conditions and communicate necessary treatment updates or concerns to care teams and families.
- Provide education to patients and families regarding disease processes, self-care, end-of-life care, and ethical considerations as part of the care plan.
- Ensure thorough, accurate, and timely documentation using EMR systems for optimal communication and compliance.
- Make appropriate referrals for additional evaluations or care when necessary, ensuring comprehensive support for patients and families.
Must-Have Skills:
- Graduate of an approved nursing education program.
- Active Registered Nurse (RN) license in the State of Maryland.
- Minimum of 1 year of clinical nursing experience (hospice environment preferred).
- Strong communication, assessment, and organizational skills.
- Current driver’s license and access to reliable transportation.
- Demonstrated compassion, dependability, and mission-driven mindset.
Nice-to-Have Skills:
- Experience working with hospice or palliative care patients.
- Familiarity with Epic or similar electronic medical record (EMR) systems.
- Experience in home health or community-based nursing care settings.
- Knowledge of end-of-life protocols, advance directives, and psychosocial support practices.
- Interest in continuous clinical education or leadership advancement within healthcare.
