We are seeking an experienced Home Health Occupational Therapist to provide patient-centered care in the home setting. This role is ideal for an OT who values autonomy, meaningful patient relationships, and collaboration with an interdisciplinary care team.

• Perform in-home OT evaluations and reassessments
• Develop and implement individualized treatment plans
• Provide skilled interventions to improve functional independence
• Educate patients and caregivers on safety and home programs
• Monitor progress and adjust plans as needed
• Complete accurate and timely documentation
• Collaborate with physicians and care team members
• Supervise Certified Occupational Therapy Assistants (COTAs)
• Maintain compliance with Medicare, HIPAA, and agency standards

• Graduate Of An Accredited OT Program
• Active Maryland OT license
• Minimum 3 Years Of Clinical Experience (home Health Preferred)
• Strong assessment and care planning skills
• EMR Experience Preferred (Well Sky/Kinnser Or HCHB)
• Reliable transportation & valid driver’s license
