Job Title: Licensed Practical Nurse (LPN)
Department: Long-Term Care (LTC) / Rehabilitation
Shift: Varies based on facility needs
The Licensed Practical Nurse (LPN) provides direct nursing care to residents in a Long-Term Care and Rehabilitation setting. The LPN works under the supervision of a Registered Nurse (RN) or physician to ensure quality patient care, support recovery, and maintain residents’ comfort and safety.
Duties and Responsibilities- Provide direct nursing care to residents in LTC and rehabilitation units.
- Administer medications and treatments as prescribed by physicians.
- Monitor residents’ vital signs and report changes in condition to the RN or physician.
- Assist residents with daily living activities and rehabilitation support.
- Maintain accurate patient records and documentation.
- Ensure a safe and clean environment for residents.
- Collaborate with healthcare team members to support patient care plans.
- Educate residents and families on care plans and health management.
- Active Licensed Practical Nurse (LPN) license in the applicable state.
- BLS (Basic Life Support) certification preferred or required.
- Experience in Long-Term Care (LTC) or Rehabilitation preferred.
- Strong communication, teamwork, and patient care skills.
