NM Nurse Practitioner (Copy)

$90 per assessment, $1,800-$9,000 per month (20-100 visits)

Wellness visits – in-home insurance assessments. No prescribing or treating.

Flexible scheduling: days, evenings, nights, weekends. Choose your days/hours, M‑Sun 8am‑7pm. Mileage reimbursed.

• Conduct adult/geriatric assessments: medical history, diagnosis, treatment, health education, referrals, case management, follow‑up, documentation.
• Collaborate with physicians, case managers, social workers, family members, key caregivers, ancillary medical personnel.
• Provide services in venues: home visits, skilled facility visits.

• BSc in Nursing preferred;
  Master’s required or commensurate experience.
• Current RN and NP licensure in state of practice with prescription authority. Board certification by AANP, AACN, or ANCC in Adult, Family Practice, Geriatrics or Adult‑Gerontology Primary Care.
• Current CPR certification.
• Valid state driver license; meet driving and liability insurance standards.
• Proficient computer skills and effective written & verbal communication.

Pay per visit $90; reimbursement for mileage; flexible hours; multiple zip codes; new grads welcome; odds 20 in‑home HRA per month.
