Mental Health Therapist

The Mental Health Therapist provides assessment, treatment, and collaboration with clients, families, and community agency staff as part of a medically integrated care team.

• Deliver a minimum number of client visits weekly and conduct mental health assessments.
• Support clients in overcoming engagement barriers by coordinating with care team members to promote whole person wellness.
• Develop and implement care plans collaboratively, maintaining clear documentation of treatment efforts.
• Participate in supervision, team meetings, and case conferences; consult with colleagues and multidisciplinary providers.
• Provide evidence-based clinical treatment and actively engage in training and consultation activities.
