As the Medical Social Worker, you provide medical social services under the direction of a physician and interdisciplinary group. You assist patients, families, and the healthcare team in understanding significant social and emotional factors related to the patient’s health status, help develop coping mechanisms, and ensure comprehensive, integrated care plans.
Primary Responsibilities- Assist the patient, significant others, physician, and healthcare team staff to understand significant personal, emotional, environmental, and social factors related to the patient’s health status on an as-needed basis.
- Contribute to the development of a comprehensive, integrated Plan of Care for patients on a daily basis.
- Instruct healthcare team members on community resources available to assist patients on an as-needed basis.
- Function as Bereavement Coordinator and supervise bereavement services, establishing a plan that addresses patient/family needs with clear delineation of services to be provided.
- Master’s Degree from a school of Social Work accredited by the Council of Social Work.
- Current CPR certification.
- Licensed Social Worker in the state of residence.
- Current Driver’s License, vehicle insurance, and access to a dependable vehicle or public transportation.
- 1+ years of social work experience in a healthcare setting.
- Bereavement Coordination experience.
- Experience establishing a plan of care for bereavement needs.
Pay is based on several factors including local labor market, education, experience, and certifications. Salary range for this role: $58,800 to $105,000 annually (full‑time). Benefits include a comprehensive benefits package, incentive and recognition programs, equity stock purchase, and 401(k) contribution, all subject to eligibility requirements.
